Dear Committee Members,

My name is Bobby McKay, student number 12345678, and my case number A9X8X60. I received a letter last week requesting information regarding my absence from last week’s exam. First, please allow me to state that I understand the gravity of the situation, and that I would like to convey my wholehearted apology to the committee, the department and most of all, to Professor Douglas.

As to the reason for my exam no-show, I needed to, unfortunately, deal with a health situation, namely an allergic reaction to medication I was taking, for which I have since been taken off by my doctor and issued with a new and hopefully non-allergenic prescription. However, last week my reaction to the medicine was so extreme that I was unable to even put on shoes as my feet had become swollen, rendering the putting on of any sort of footwear impossible.

I have attached photos of the medication, a doctor’s note explaining the allergic reaction, and time-stamped photos of the reaction I had suffered.

Again, I deeply apologize for the incident and sincerely request that the Commission review the decision in the hope that I may have another opportunity to sit the exam.
